Attention Parents of Seniors: All immunization records were mailed home to students either with final report cards (Boys Division) or in a separate mailing (Girls Division). We do not keep any copies of immunization records after students graduate.
Attention Parents of Returning Students: All health forms will be available in hard copy through the summer mailing or you can download them below. It is mandatory that new forms be completed for each student at the beginning of each school year. These forms, as well as prescribed and over-the-counter medications, will be collected during registration in August. Remember to have the “Prescription Medication Form” signed by a doctor
in order for medications to be given at school. Please contact the school nurse if you have any questions.
Summer mailing
Last chance reminder: we would like to remind parents to schedule doctor appointments for athletic physicals and
immunizations. In accordance with Colorado Immunization Laws, all students attending school must have a complete
immunization record on file with the school they currently attend.
The state is requiring that students receive a Tdap booster by their sophomore year of high school. The Tdap booster is a combination of Tetanus Toxoid, Diphtheria Toxoid and Acellular Pertussis (Whooping Cough). Varicella booster (Chicken Pox vaccine)*, Menactra (Meningococcal vaccine), and Hepatitis A (series of two shots) are also being recommended at this time. If your son/daughter receives a physical this summer, please check with the physician to see if he/she is current on immunizations and request an updated immunization record at that time.
Please submit a copy of the student’s updated immunization information to the school nurse if you have not already done so.
*Varicella booster is a requirement, if your son/daughter has never had Chicken Pox disease, for incoming freshmen and sophomores in the 2010-2011 school year.
Asthma, Food Allergies and Anaphylaxis Management: There is a new state act concerning policies for the management of asthma, food allergies, and anaphylaxis among school children, According to Senate Bill 09-226:
Prior to the beginning of each school year, parents of students with asthma, known food allergy, and anaphylaxis must provide the following to the school’s administration and health care provider:
1. Documentation regarding the diagnosis and history of the student’s food allergy or asthma.
2. Identification of all foods to which the student is known to be allergic.
3. Identification of any medication prescribed for the student for the treatment of a food allergy or anaphylaxis.
4. Any specific signs or symptoms that may indicate the student is having an allergic reaction to a food.
5. Emergency treatment procedure to employ in the event that the student suffers.
6. The names and telephone numbers of persons whom the administration of the student’s school should contact in addition to emergency medical personnel in the event that the student suffers an allergic reaction to food.
7. The name, telephone number and signature of the student’s primary health care provider.
Parents and legal guardians of students for whom the medication has been prescribed for treatment of a asthma, food allergy or anaphylaxis are encouraged to give the school nurse a supply of the medication unless the student has an approved treatment plan pursuant to section 22-1-119.5that authorizes the student to self-administer the medication.
Furthermore, parents and legal guardians are asked to discuss the above with your son's or daughter’s primary health care provider, complete necessary paperwork requiring physician’s signature and return forms and medication to the school nurse prior to the beginning of each school year. The nurse will be available at registration to accept paperwork and medications. Please make sure medications include the original pharmacy label identifying the student, medication, dosage and physician information, as well as a current expiration date.

Gently-Used Clothing: Has your son/daughter outgrown some of his/her khaki pants or collared shirts? Are you aware that the BD and GD Clinic and Dean’s Office lends out shirts, pants, ties, and belts throughout the school year to students in need? Our supply has dwindled, therefore we are taking up a collection. When cleaning out closets this summer, please consider donating gently-used pants, shirts, ties, and belts. Please place donations into the black bin outside the BD Clinic or turn into the GD Main Office.
